Joy Tree Child Entry Form

Submit your CASA child's information securely to CASA of Lexington so they can receive Joy Tree gifts this year. This opportunity is available to child(ren) currently served through your CASA case. Siblings or other children involved in the child welfare system are not eligible. Submissions must be received by Friday, Oct 16, 2026. No late submissions will be accepted.

Submit only the child's first name. Children to whom you are assigned as a CASA volunteer are eligible to participate. Siblings and/or other children involved in the child welfare system are not eligible.

Child's Wish List

Enter three "wants" and three "needs" your child has below. Please list tangible items across a variety of price ranges, including at least one wish under $50. Do not list gift cards or cash as a wish. Please include clothing and shoe sizes, noting toddler, youth, junior, or adult sizing as appropriate.

Choose Your Pickup Time

Choose a date and time when you will pick up the gifts purchased for your CASA child through the Joy Tree program. Pickups will take place only during scheduled pickup windows this year to help us better support all participating families.
Available Pickup Times(Required)
The Fayette County CASA office is located at 3245 Loch Ness Dr. in Lexington. The Lincoln County CASA office is located at 201 E. Main St., No. 8 in Stanford. While you can designate someone else to pick up your CASA child's gifts, only CASA volunteers sworn on the child's case may deliver them.
